This is done on purpose. We understand that many patients may choose to have a facelift at some point in the future, and deep placement allows Sculptra® to support the face without interfering with surgical planes. In fact, when Sculptra® is placed in the deep fat pads, it does not add complexity to a facelift procedure. It actually helps.
When Sculptra® is placed more superficially, such as in the subdermal or deep dermal plane, it can be visible to a surgeon and may require additional work during tissue dissection. Even in those cases, it does not mean a facelift cannot be performed. An experienced plastic surgeon can still safely proceed; it simply requires more careful dissection.

A simple approach is to massage for a couple of minutes during your normal skincare routine, such as once in the morning and once in the evening after washing your face. Some patients prefer to follow the traditional five minutes, five times a day, for five days approach, which is also acceptable.
Ultimately, there is flexibility with massage after Sculptra®. Whether you choose to massage or not, your provider will guide you based on your treatment and comfort level.
It is normal to feel temporary firmness or small bumps in the treated areas shortly after Sculptra® treatment. This is related to the product itself and the skin’s normal healing response.

Because of this deep placement, it is very uncommon for patients to feel any nodules. If a small area of firmness were to develop, it would typically be deep and not visible to others.
While nodules are uncommon overall, placement in more superficial tissue planes can make any firmness or irregularity more noticeable, which is why our approach focuses on deeper anatomical placement. Modern dilution techniques have further reduced the likelihood of visible irregularities.
Bougie Aesthetics® also serves as a complications referral clinic, and we routinely evaluate and treat patients who have developed concerns from Sculptra® treatments performed elsewhere. Our team is highly trained in identifying and managing nodularity when it occurs.
If you ever notice an area of firmness that concerns you, we encourage you to return for evaluation so it can be addressed appropriately.
The first and most important question is whether the area is visible or simply something you can feel.
If you can feel a small firmness but it is not visible, this is usually not a concern. In many cases, this represents localized collagen formation where the product stimulated collagen more focally. If anything, this can be a sign that your body responds well to Sculptra®. These areas are typically deep, not noticeable to others, and often soften over time.
If a nodule is visible, this usually indicates placement in a more superficial tissue plane. While this is uncommon, especially with modern dilution techniques, it can occur. When visible nodularity does develop, it can be safely evaluated and treated.
